I understand the responsibilities and commitments involved in this leadership role and pledge to carry out my duties with dedication and integrity.I Have Read and UnderstoodAs a Youth Leader, I acknowledge and commit to the following responsibilities: Student Support: I will provide guidance, support, and encouragement to the students attending the Youth Camp. I will prioritize their safety, well-being, and inclusivity during all camp activities. Role Model: I will strive to be a positive role model for all camp participants by exemplifying respect, kindness, and responsibility. Collaboration: I will work collaboratively with fellow Youth Leaders, Camp Organizers, and Camp Staff to ensure the smooth execution of the camp program. Activity Supervision: I will supervise and actively participate in the camp activities to ensure they are conducted safely and in accordance with the camp guidelines. Conflict Resolution: In case of any conflicts or concerns among students, I will address the issues promptly and respectfully, seeking appropriate assistance if necessary. Emergency Preparedness: I will familiarize myself with emergency procedures and ensure that all students are aware of safety protocols during the camp. Communication: I will maintain open and transparent communication with students, parents, and camp organizers throughout the duration of the camp. Attendance: I will be punctual and present during all scheduled camp activities and events. Compliance: I will adhere to all rules, policies, and guidelines set forth by World Harvest Church and the Youth Camp. I understand that my role as a Youth Leader is essential to the success of the Youth Camp and that any breach of this agreement may result in my removal from the leadership position. By signing below, I affirm that I have read and understood the responsibilities outlined above and agree to fulfill them to the best of my abilities.I Have Read And UnderstoodI understand that the risks associated with camp activities could result in injury. I hereby assume these risks. As a part of this activity, I understand it will be necessary to engage in travel. I hereby grant my permission for such travel. I understand that the World Harvest Church is not liable for any injuries or other occurrences due to indoor and outdoor camp activities or related risks, or the actions or omissions of WHC counselors, volunteers, employees, or any other entities being released.I Have Read and UnderstoodIn an emergency, I agree to pay any necessary expenses incurred in my medical treatment, including, but not limited to, all transportation costs to and from a medical facility and, if necessary, transportation to my home or medical facility of choice.I Have Read and UnderstoodIn the case of an emergency affecting my health or welfare, the counselors, leaders, or adult chaperones have permission to administer first aid and or transport me to the nearest doctor or hospital for further emergency medical attention as deemed necessary. The individual acting in response to the emergency, World Harvest Church, and its representatives will be held blameless.I Have Read and Understood Below are the parameters for communication with youth, * Youth leaders will only initiate one conversation per week (does not include messages for scheduling purposes). * Youth leaders will only communicate with a minor child via phone calls, emails, text messages, and ChurchCenter. A minor child may interact with a youth leader on a social media platform, but DM’s are not allowed. If a minor child DM’s a youth leader, the youth leader will direct the conversation to text messaging. * All text message communication must include the WHC Student phone in the thread. This will create a Group Chat between the minor child, the youth leader, and the church phone. * Youth leaders are only to communicate with a minor child Monday through Friday between the end of the school day and 9:00 PM. Youth leaders may communicate with a minor child Saturday & Sunday, 8:00 AM – 9:00 PM. * Youth leaders are not counselors; they are mentors and cannot legally provide counseling. WHC offers counseling services if they are required. * If a text message conversation progresses into a deep conversation where the minor child is asking for advice, the youth leader will direct the conversation to a phone call or an in-person meeting at the next church service. Text messaging does not include the body language or tone of voice that is vital to understanding advice given by the youth leader. * All phone conversations must be kept to a maximum of 30mins. * No youth leader should engage in messaging via apps with disappearing messages, ie. Snapchat, Instagram, etc. I understand and acknowledge that I am fully aware of and assume the risks including but not limited to the risk of the said minor child’s participation regarding communication with WHC Youth small group leaders. I recognize my responsibility to ensure that said minor child participates only with the WHC Youth leaders they are assigned to for communication regarding WHC Youth events and small group activities. I further understand and agree that WHC disclaims all responsibilities, express or implied, regarding the minor child corresponding outside the limits of prescribed above. Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s communication with WHC Youth leaders as long as it pertains to WHC Youth events and small group activities. To the fullest extent allowed by law, I hold harmless and agree to indemnify WHC, its officers, directors, staff, volunteers, employees, and agents from and against any present or future claim, cause of action, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in, communication with unauthorized individuals regarding WHC Youth events and small group activities, regardless of fault. GABB Phone Waiver My minor child has a GAAB Phone and cannot message in group texts, this includes group texts with the WHC Student Number. I give permission for my student to message leaders without using the WHC Student Number, as I can monitor all messages on my minor child's phone.I Have Read and Understood I am at least eighteen years of age and have carefully read and freely signed this Liability Waiver and Release Form (Minor Child). I understand and agree that no oral or written representations can or will alter the contents of this document.
