IMPORTANT INFORMATION: BEFORE PROCEEDING TO APPLICATION, PLEASE READ:
World Harvest Church has a heart to help and serve our community. However, due to the impact of our current economy and the high volume of requests from our congregation, we are only potentially able to provide financial assistance to those actively connected to World Harvest Church through:
– Membership/regular attendance of weekend services for at least 3 months
– Serving others through one of the ministries of World Harvest Church
If you feel you meet the above criteria, please fill out the form below and click submit when finished. Please note that meeting the criteria above does not guarantee assistance. If you do not qualify, we encourage you to download the Resource List from the Support Resources page on our website for more options.
In order for your application to be considered, you must submit copies of your driver’s license, copies of the current bills for which you are requesting assistance and, if requesting rent assistance, your current lease. (If you pay your rent to an individual we need the landlord to complete and sign a W9 form and e-mail or fax to us).
The names and addresses on your bills and on your license must match the address provided on your application.

*Regardless of the urgency of your request, your application will not be processed until all accompanying documentation is received.
Once we have received your documentation (Financial Assistance Form, Driver’s License and copies of all bills,
DO NOT send Social Security Information) please be patient, it may take up to 5-7 business days to process your request. In the meantime, we encourage you to download the Resource List from the Pastoral Care page on our website for more options.
